Once
this form is submitted you will be automatically issued an RMA number.
This RMA number will be issued when your media is received. Please
label your shipping box with GINRMA on the outside of the box, and on your drive with your name, telephone number.
This form will
combine the information collected by our technician, during your telephone
interview, with your choices. This combined information will be used
to determine which of our offices can provide you with the best recovery
and support, during this process.

Please enclose your drive in an anti-static bag, or wrap in foil.
Shock protect your drive, in either a drive retail box, or wrap in bubble wrap, dense
foam, or crumpled paper. Ensure that the packing is at least 1" thick on
all sides, and that the drive can not shift in the package.

Media submitted with invalid payment, or
invalid return address information will be held until valid payment or
address information is provided. Media submitted with
invalid information will be held for 30 days, and after that point
will be destroyed or discarded.
2.
Media, drives or diskettes, submitted with improper packaging will
be accepted, but the minimum success rate may be affected.
This is because, improper packaging can further damage your media, and
may ruin your chances of successful recovery.
3.
Media can be returned by whatever method you describe.
However, you will be charged the full insured shipping fee..
4.
Data recovery is a service, not a product. This means that
we will always tell you the potential for success, before we accept
your payment for a particular procedure.
All payments
for service are non-cancelable.
5. Data
recovery is a destructive process, unless you have written
notification, your media will be subjected to various physical and
software processes which may render it un-usable.
However,
all drive manufacturer warrantees
are protected through out our procedures.
By
submitting your media you explicitly agree to these procedures.
6. The minimum
costs of recovery, unless specified in writing, is
$400.
USD. Although some discounts may apply, for non-profit
institutions, and students. All data submitted for recovery must
be worth at least this minimum. Media submitted, and
successfully recovered is a costly, time consuming process, this is
explicitly agreed by your submission. Media successfully recovered
will obligate the submitter to a minimum cost of $500, less discounts.
Payments less than the minimum will not be accepted as full payment,
and those whom submit such payments will be pursued by our legal and
collection departments. The only exceptions to this will be when
the time stated for recovery, by the customer at the time of
submission are exceeded.
7. Warrantees of
drive enclosures, such as USB cases, or network appliance cases, are
not protected. Cases must be opened in order for
your drive to be recovered. In actual practice, the cases
are rarely damaged, however most manufacturer warrantees do not allow
the case to be opened.
All Data Recovery
is considered to be assigned risk or experimental by the
manufacturers!
Submission
of your media, is explicit agreement to be bound by our conditions of
service.
